چکیده

to the editor: The report by Toff and colleagues on the United Kingdom Pacing and Cardiovascular Events (UKPACE) trial (July 14 issue), 1 evaluating cardiovascular end points in elderly patients with atrioventricular block, showed that the use of dual-chamber pacing to maintain atrioventricular synchrony conferred no survival advantage over the use of single-chamber pacing. These results may be considered along with those of recent trials involving patients with atrial fibrillation that showed a lack of mortality benefit associated with a strategy that maintains atrioventricular synchrony by restoring sinus rhythm. 2 Perhaps the consequences of atrioventricular synchrony deserve closer inspection. As Ellenbogen and Wood mention in the accompanying editorial, 3 atrioventricular pacing increases stroke volume and systolic blood pressure, as compared with ventricular pacing. These effects are probably more pronounced in elderly persons because of reduced aortic compliance and an increased incidence of diastolic dysfunction. Did the different pacing strategies lead to a meaningful difference in blood-pressure control or medication requirements? The harmful effects of higher blood pressure in the dual-chamber group may have offset any benefit from this pacing mode. In addition, how was baseline hypertension ascertained? A prevalence of 33 percent is about half what one would expect for an elderly population.

Benefits of dual chamber pacing in sick sinus syndrome.

The effects of DDD (fully automatic) and VVI (ventricular demand) pacing modes on exercise tolerance, symptom diary cards, and Holter monitoring were investigated in a randomised double blind crossover study of 16 patients who had had DDD pacemakers implanted because of frequent syncope. Eight patients presented with sick sinus syndrome and, with one exception, retrograde atrioventricular condu...

Usefulness Of Ivabradine To Treat "unexpected" Heart Failure Caused By "acute" Right Ventricular Pacing

We present the case of a patient with a heart failure episode induced by acute right ventricular pacing. After reversal of beta-blockers because of chronic obstructive pulmonary disease (COPD) exacerbation, the following sinus tachycardia caused a 2:1 atrioventricular block and consequent continuous right ventricular pacing. He was treated with the selective I(f) inhibitor ivabradine, that redu...

Successful electrical pacing for complete heart block complicating diphtheritic myocarditis.

A case of severe diphtheria complicated by myocarditis and neurorespiratory paralysis is reported. The myocarditis manifested with severe conduction disturbances including left bundle-branch block and high grade second degree atrioventricular block leading to Adams-Stokes attacks. Temporary transvenous electrical pacing for.3 days was successful in the management of this complication, but posit...

Second-degree atrioventricular block type II and third-degree atrioventricular block requiring cardiac pacing after tooth extraction

Because atrioventricular block can cause cardiovascular collapse, the rapid management of this condition is necessary. An 82-year-old man complained of discomfort after tooth extraction, and electrocardiography showed second- and third-degree atrioventricular blocks. Dentists should monitor patients who report discomfort using electrocardiography because of the possibility of life-threatening a...

Double blind crossover comparison of the effects of dual chamber pacing (DDD) and ventricular rate adaptive (VVIR) pacing on neuroendocrine variables, exercise performance, and symptoms in complete heart block.

OBJECTIVE To compare the effects of dual chamber pacing (DDD) and ventricular rate adaptive pacing (activity sensing) (VVIR) in patients with complete heart block. DESIGN Double blind crossover comparison with one month in each pacing mode. PATIENTS 10 consecutive patients aged 23-74 presenting with complete anterograde atrioventricular block at rest and on exercise and with an intact atria...
